Health coaches help you focus on behavior changes in four key areas: nutrition, exercise, sleep, and stress management. They work with you and your care team to develop a plan that fits your lifestyle to help improve your overall quality of life.
Sometimes the medical advice you get from your provider can seem overwhelming or confusing. The Harbor Health coaches take the information you receive in the clinic and help you apply it to your everyday life. They serve as a line of communication to help you better understand how to achieve clinical goals in a realistic and individualized way through behavior change.
They understand this behavior change isn’t linear and there will be ups and downs. They’re patient and take an encouraging approach to help you navigate the waters of change. And they don’t take a rigid attitude that requires you to eliminate certain foods completely or have a super intense exercise routine.
Health coaches can be beneficial for people who are focused on weight management, diabetes management and prevention, balancing cholesterol, and lowering blood pressure. Harbor Health even has health coaches who work with parents who need support innutrition, exercise, and creating healthy sleep routines for their children.
How Does the Process Work With a Health Coach?
Our Harbor Health coaches typically start by discussing your current health status, lifestyle habits, and personal goals. Based on this information, we develop a customized plan that is tailored to your needs and preferences. We then work with you to implement the plan, providing ongoing support and accountability to help you stay on track. The ultimate goal is to help you achieve optimal health and wellness, both physically and mentally.
The good news is we won’t tell you what to do. Instead, we empower you to come up with solutions that integrate well into your life. Often times, when you go to your provider, you are given instructions and sent out on your own to implement them. By providing an extra layer of support from a health coach, we may improve clinical outcomes, keeping your care team connected to you as you make those changes.
